Privacy Policy
This Policy explains information practices across the InnerVital public website, nonclinical Membership, botanical commerce, and payment/provider routing.
1. Scope and responsible entities
InnerVital Health Group, Inc. operates innervital.com and the nonclinical InnerVital Membership. InnerVital Botanical Supply Inc. is the seller for botanical ecommerce and supported point-of-sale transactions. Independent professional practices, including InnerVital Wabash PLLC and InnerVital Berwyn PLLC, control information collected for their own clinical care, billing, and patient records. The entity identified when information is collected is responsible for that activity, subject to applicable law and agreements.
2. Information the public website may collect
Depending on how you interact with the website, we may collect name, email address, phone number, postal code, preferred location, organization, role, inquiry category, general service interest, communication preference, form messages, webinar or event registration, career materials, and other information you choose to provide. Public forms are not designed for clinical intake or payment-card data.
3. Account and Membership information
For InnerVital Membership, InnerVital Health Group, Inc. may collect identity and contact information, billing address, Membership status, enrollment and Terms-acceptance records, paid-through and renewal dates, benefit eligibility, communication preferences, cancellation requests, support history, and transaction or subscription information.
Processor-hosted checkout may provide Stripe customer, checkout-session, subscription, invoice, payment, refund, or dispute identifiers and limited payment-method descriptors such as card brand and last four digits. The integration is designed so InnerVital does not receive or store full card numbers, security codes, or un-tokenized payment credentials when processor-hosted Stripe Checkout or Payment Links are used.
4. Botanical ecommerce and point of sale
InnerVital Botanical Supply Inc. and Shopify may process product selections, customer identity and contact information, billing and shipping addresses, fulfillment preferences, order history, payment and transaction status, discounts, returns, refunds, fraud signals, support messages, and point-of-sale information. Shopify and connected commerce providers process information under their own privacy terms and their services to the merchant.
5. Clinic and professional-practice information
A professional practice may use its own Stripe account, eClinicalWorks, Healow, or another clinical/payment system for scheduling, intake, patient records, insurance, billing, refunds, and disputes. Information collected in that workflow is governed by the practice's notices and applicable healthcare/privacy law. InnerVital Health Group, Inc. does not become the clinical provider or merchant merely because the website routes you to that system.
6. Device, security, and analytics information
Hosting, content-delivery, security, form, and service providers may receive IP address, browser and device details, referring page, requested page, timestamps, error and security events, cookie or similar-technology identifiers, and general usage information. The current protected New Patient Intake is configured without third-party advertising analytics. If analytics or cookie practices materially change, this Policy and any required consent controls must be updated before deployment.
7. Cookies and similar technologies
The website and connected services may use essential cookies, local browser storage, or similar technologies for security, preferences, session continuity, checkout, and service operation. Stripe, Shopify, Google scheduling, and other linked services may use their own technologies when you visit them. Where applicable law requires consent for nonessential technologies, appropriate choices will be provided.
8. How information is used
Information may be used to provide and secure the website; respond to inquiries; route users to the correct seller or professional practice; administer Membership and benefits; process and reconcile transactions, subscriptions, refunds, disputes, taxes, and cancellations; fulfill botanical orders; prevent fraud; maintain records; improve accessibility and service quality; send operational, renewal, security, or policy notices; honor communication preferences; enforce terms; and comply with law.
9. Merchant/provider routing and affiliated entities
Information may be routed among InnerVital-affiliated entities only as reasonably necessary for the request or transaction and subject to role, authorization, contracts, and law. A Membership purchase is routed to InnerVital Health Group, Inc.; a botanical purchase to InnerVital Botanical Supply Inc.; and a clinical request or payment to the identified professional practice. We do not intentionally combine Membership payment identifiers with clinical/PHI data unless a lawful, documented workflow requires the minimum necessary connection.
10. Service providers and third parties
Information may be processed by hosting, security, communications, form, analytics, customer-support, payment, accounting, fraud-prevention, fulfillment, shipping, ecommerce, scheduling, and clinical-system providers. Current or anticipated examples include Netlify, Stripe, Shopify and Shopify POS, Google scheduling, eClinicalWorks, and Healow. Providers process information under contracts, their own terms, and applicable law.
11. Marketing communications
Where permitted, we may send education, event, Membership, service, botanical, or location updates. You can unsubscribe using the method in the message or request a preference change through Contact InnerVital. Opting out of marketing does not stop necessary transaction, security, cancellation, renewal, or policy notices.
12. Sharing and disclosures
We do not intend to sell information submitted through general website forms. Information may be disclosed to the responsible InnerVital entity, professional practice, service provider, advisor, transaction party, or authority as needed to fulfill a request, administer a transaction, investigate fraud/security issues, reconcile payments, handle refunds/disputes, protect rights and safety, complete a business transaction, or comply with law. Jurisdiction-specific “sale” or “sharing” definitions may provide additional rights.
13. Retention
Information is retained for as long as reasonably necessary for the purpose collected, the relationship or transaction, security, fraud prevention, support, accounting, tax, dispute, legal, backup, and recordkeeping needs. Retention differs by data type and responsible entity. Professional-practice clinical records follow separate legal and professional requirements.
14. Security
We use administrative, technical, and organizational safeguards appropriate to the information and service. Processor-hosted checkout is preferred so the public website does not handle full card data. No system is perfectly secure. Do not send sensitive information through a channel that does not request it.
15. Children
The general website and nonclinical Membership are not directed to children under 13. A parent or authorized representative should use public forms where appropriate. Clinical services for minors, if offered, follow the professional practice's consent and privacy procedures.
16. Privacy rights and choices
Depending on where you live and subject to exceptions, you may have rights to request access, correction, deletion, portability, restriction, or information about certain disclosures, or to opt out of certain marketing, targeted advertising, sale, or sharing. Submit a request through Contact InnerVital and identify the relevant relationship (website, Membership, botanical order, or clinic). We may verify identity and route the request to the responsible entity. You may appeal or contact a regulator where applicable law provides that right.
17. External services and international processing
Third-party services may process information in other states or countries under their own safeguards and legal obligations. Their privacy policies apply to their direct services. Review the notice presented by Stripe, Shopify, Google, eClinicalWorks, Healow, or another provider before continuing.
18. Changes to this Policy
We may update this Policy as services, entities, vendors, laws, and data practices change. The effective date will be updated, and material notice or consent will be provided when required.
19. Contact
For general website or nonclinical Membership privacy questions, use Contact InnerVital or write to InnerVital Health Group, Inc., 211 W Wacker Dr, Suite 120-2389, Chicago, IL 60606. For botanical orders, contact InnerVital Botanical Supply Inc. through the shop/order channel. For clinical information, contact the professional practice identified in your care documents. Do not send PHI or full card information through general email or contact forms.
