Effective August 29, 2026
Terms of Use
These Terms explain the role of the InnerVital website and the separate legal entities that sell Membership, botanical products, and professional clinical services.
1. Acceptance of these Terms
By accessing innervital.com, submitting a public form, using an InnerVital digital tool, or proceeding to a transaction offered through the website, you agree to these Terms and any additional terms presented for the particular Membership, product, or professional service. If you do not agree, do not use that feature or complete the transaction.
2. InnerVital website and platform role
Innervital.com is an umbrella brand, information, navigation, and technology layer operated by InnerVital Health Group, Inc. The website may help visitors learn, locate services, prepare questions, review nonclinical Membership benefits, or continue to a separate seller's or provider's system. A common brand does not make every InnerVital entity the same legal seller or provider.
3. Legal entity architecture
- InnerVital Health Group, Inc. offers the optional, nonclinical InnerVital Membership and operates the nonclinical website/platform layer.
- InnerVital Botanical Supply Inc. sells botanical products through the InnerVital Botanical Shop and Shopify, including supported point-of-sale channels.
- Independent professional practices, including InnerVital Wabash PLLC and InnerVital Berwyn PLLC, provide and bill their own clinical services. The applicable practice is identified before clinical scheduling or payment.
These entities may share brand, administrative, or technology resources where lawful, but seller, provider, merchant, receipt, refund, and patient-financial responsibilities remain with the entity identified for the transaction.
4. Informational and wellness content
Website articles, educational materials, Traditional Chinese and East Asian Medicine content, Membership resources, and general wellness tools are for education and navigation. They are not a substitute for an individualized evaluation by a qualified professional.
5. No medical advice or provider-patient relationship
Browsing the website, using a public form, receiving general information, or using an AI-supported wellness/navigation tool does not provide medical advice, diagnosis, treatment, or emergency services and does not by itself create a provider-patient relationship. A clinical relationship, if any, is established separately by the professional practice under its intake, consent, and care procedures.
6. Emergencies
Do not use this website, public forms, Membership support, or AI tools for an emergency. Call 911 or the applicable local emergency number, or seek immediate in-person care.
7. AI wellness and navigation tools
AI-supported features may help organize user-provided information, questions, or general next steps. Outputs may be incomplete or inaccurate and require human judgment. Do not rely on them as a diagnosis, clinical decision, safety determination, or substitute for a licensed professional. Additional privacy and workflow notices presented with a protected tool also apply.
8. InnerVital Membership
InnerVital Membership is an optional, nonclinical program sold by InnerVital Health Group, Inc. The current offer is $125 USD per year. Membership may include 15% savings on eligible InnerVital Botanical products, AI wellness tools, seasonal TCM nutrition/lifestyle content, education, webinars, promotions, and other genuinely nonclinical benefits described at enrollment.
Membership is not insurance, does not include clinical care, does not provide a treatment discount, and cannot waive deductibles, copayments, coinsurance, or other required patient responsibility. InnerVital Health Group, Inc. does not compensate for patient referrals.
9. Automatic renewal and billing authorization
The annual Membership automatically renews each year until canceled. Before enrollment, the price, renewal cadence, cancellation deadline, and clinical-care exclusion must appear clearly near the purchase action. By completing processor-hosted checkout, you authorize the seller and its processor to charge the disclosed initial and recurring amounts until cancellation, subject to applicable law.
For automatically renewing annual Memberships, InnerVital Health Group, Inc. will provide renewal notices required by applicable law using the contact information associated with the Membership. Keep your contact information current and review each notice for the renewal date, price, and cancellation instructions.
10. Membership cancellation and refunds
You may cancel online at innervital.com/membership/cancel/ before the renewal deadline to avoid the next annual charge. Unless the enrollment offer or applicable law states otherwise, cancellation takes effect at the end of the current paid period and benefits remain available through the paid-through date. Cancellation does not automatically create a prorated refund. Refund requests are reviewed under the policy presented at purchase and applicable law. See the Refund & Cancellation Policy and Membership Terms.
11. Clinical services and payments
Clinical services are not sold by InnerVital Health Group, Inc. Clinical services are provided by the professional practice location that is selected to provide the service. Each clinic location sets clinical pricing and patient-financial policies, and is the clinical merchant. Clinical payment or scheduling may continue to a practice-specific Stripe, EMR account, or another approved system.
A Membership receipt must never be represented as a clinical receipt.
Booking, cancellation and rescheduling policy
Each appointment follows the price and policy you accepted when booking.
When to cancel or reschedule
- At least 24 hours before the original appointment start: Your change is considered on-time. Exactly 24 hours counts as on time.
- Less than 24 hours before the original appointment start: Your change is considered late.
You cannot cancel or reschedule an appointment once the scheduled appointment time has started.
If you choose to Pay Later
Nothing is due when you book. Your visit amount is due when your appointment starts, even if you do not attend. Your authorized saved card will be charged at that time or afterward. Processing may be delayed.
- Cancel or reschedule on-time: You owe nothing for the original appointment, and there is no change fee.
- Cancel or reschedule late, before the appointment starts: You do not pay the original appointment amount. Instead, you pay $82.50 for an in-person appointment or $42.50 for telehealth instead of the original visit amount. We charge your authorized saved card after accepting the change.
- When you reschedule: The new appointment has its own displayed price, due when the appointment starts.
Treatment credits cannot be used when Pay Later option is selected.
If you choose to Pay Now
You pay when you book and payment is non-refundable and can only be rescheduled as follows.
- Reschedule on-time: Your payment transfers to the new appointment with no change fee.
- Reschedule late, before the appointment starts: Your payment transfers to the new appointment, and you pay a change fee equal to 50% of your original payment. This fee is rounded to the nearest cent, with half cents rounded up.
- Canceled or missed appointments: Your payment is forfeited and rescheduling is not possible.
If you use Treatment Credits
Care Plan purchases cannot be refunded for cash.
- Cancel or reschedule on time: All credits used for the appointment are returned to your account.
- Cancel or reschedule late, before the appointment starts: Half the credits are returned, and half are forfeited. For example, an 8-credit in-person appointment returns 4 credits; a 4-credit telehealth appointment returns 2.
When you reschedule, the new appointment requires its full displayed number of credits.
Your available credits, including those being returned, must cover the new appointment. If you do not have enough credits or the new time is unavailable, your original appointment and credit balance stay unchanged. Cash cannot replace insufficient credits.
When your appointment starts
Once your appointment’s scheduled start time arrives:
- You can no longer cancel or reschedule it.
- The full appointment amount is due unless you have already paid or used Treatment Credits.
- You have 15 minutes to arrive. If you arrive late within this period, your treatment time may be shortened so the next patient’s appointment can start on time.
- If you do not arrive within 15 minutes, your appointment is considered missed. Your payment or Treatment Credits are forfeited and there can be no refund or credit return.
- If you need another appointment, you must book it separately and pay the normal appointment price or use the required Treatment Credits to book.
Attendance is recorded separately from payment. An appointment is not marked attended simply because its scheduled time has passed.
Questions or help
Submit a support case through Support in your patient portal. Support cases are usually answered within 24 business hours. Requests for exceptions require clinic approval.
Submitting a support case does not cancel or reschedule your appointment. To avoid late-change penalties, complete your cancellation or rescheduling at least 24 hours before the original appointment start time.
Optional experience survey
If you opt-in, you may receive one optional experience survey after your appointment ends.
Clinic Policy for Acupuncture / TCM and Massage & Manual Therapy
Late arrival: Appointment extension time may not be possible. Late arrival may reduce the available hands-on session time so the next patient's appointment is not delayed.
- 24 hours or more before appointment: Cancel or reschedule without a late-cancellation fee.
- Less than 24 hours: A late-cancellation fee of up to 50% of the scheduled session price may apply, subject to reasonable emergency exceptions.
- No-show: The scheduled session fee may be nonrefundable / charged, subject to reasonable emergency exceptions.
- Clinic cancellation: If the clinic cancels the appointment, the patient may reschedule without charge or receive a refund of any amount actually paid.
- Duplicate charge: Duplicate charges are refunded.
- Clinical contraindication before service: If the professional practice determines before substantive service begins that the session should not proceed for clinical/safety reasons, any applicable session payment should be refunded or credited according to the clinic's approved policy.
12. Botanical ecommerce
Botanical products are sold by InnerVital Botanical Supply Inc. through the InnerVital Botanical Shop, Shopify, and supported Shopify point-of-sale channels. A Membership discount applies only to eligible items and does not make InnerVital Health Group, Inc. or a professional practice the botanical seller. Product availability, safety information, taxes, shipping, returns, and refund terms shown by the botanical seller also apply.
13. Shipping and returns
The Shipping & Returns Policy is incorporated into these Terms for botanical purchases. Product-specific restrictions disclosed before purchase may apply, particularly to opened, perishable, personalized, or safety-sensitive items, subject to applicable law.
14. Third-party payment and service providers
Transactions may be completed on third-party platforms. Stripe may process Membership or separately configured professional-practice payments. Shopify supports botanical ecommerce and point-of-sale transactions. eClinicalWorks, Healow, or successor systems may support clinical scheduling, patient intake, or practice payments. Those services have their own terms and privacy practices. The legal entity identified at the transition remains the seller or provider unless the transaction screen expressly states otherwise.
15. Pricing, corrections, taxes, and promotions
Prices, eligibility, availability, taxes, shipping charges, and promotional conditions are shown before purchase where known. InnerVital and the applicable seller/provider may correct good-faith errors before fulfilling a transaction, provide notice where required, and offer a lawful cancellation or refund option when appropriate. Discount codes and promotions are subject to stated eligibility, exclusions, expiration, non-transferability, and applicable law. Tax collection is configured by the responsible seller or provider.
16. Communications and electronic notices
You consent to receive transaction, account, policy, security, cancellation, renewal, and other service notices electronically at the contact information you provide, subject to applicable law. Marketing communications are optional where required and include an opt-out method. Keep your contact information current and retain transaction confirmations and renewal notices.
17. Intellectual property
InnerVital names, marks, site design, original content, software, and related materials are owned by or licensed to the applicable InnerVital entity and are protected by law. You may use the website for personal, lawful purposes but may not copy, sell, scrape, reverse engineer, or exploit protected materials except as permitted by law or written authorization.
18. Prohibited use
Do not misuse the website; interfere with security or availability; upload malware; impersonate another person; submit information you lack authority to share; use public forms for PHI, urgent medical issues, or payment-card data; attempt unauthorized access; or use content to provide unlawful, misleading, or unsafe services.
19. Third-party links
Links to outside websites are provided for convenience or to complete a clearly disclosed transaction. InnerVital does not control third-party content, availability, accessibility, or security and is not responsible for it except to the extent required by law.
20. Disclaimer of warranties
To the extent permitted by law, the public website and general informational tools are provided “as is” and “as available.” InnerVital does not guarantee uninterrupted access, error-free content, a particular result, provider availability, insurance coverage, appointment confirmation, or that general wellness content is appropriate for a particular person. Non-waivable warranties and consumer rights remain unaffected.
21. Reasonable limitation of liability
To the extent permitted by law, an InnerVital entity is responsible only for its own acts, omissions, and transaction obligations and is not responsible for another legal entity's professional judgment or merchant obligations merely because the InnerVital brand is shared. Nothing in these Terms excludes liability or remedies that cannot lawfully be excluded or limited.
22. Applicable law and disputes
Applicable law governs these Terms and each transaction. Nothing here limits non-waivable rights or remedies. Entity-specific purchase or clinical terms may include additional lawful dispute procedures disclosed before the transaction. These Terms do not impose a new mandatory arbitration requirement or class-action waiver.
23. Changes to these Terms
We may update these Terms as services, laws, vendors, or business operations change. The effective date will be updated, and material changes will be communicated when required. Changes to Membership price or renewal terms apply only with legally required notice and consent.
24. Contact
For website, nonclinical Membership, or corporate questions, use Contact InnerVital or write to InnerVital Health Group, Inc., 211 W Wacker Dr, Suite 120-2389, Chicago, IL 60606. For botanical orders, use the support channel identified by InnerVital Botanical Supply Inc. in the shop/order confirmation. For clinical care or billing, contact the professional practice identified in the location or transaction workflow. Do not send PHI or card information through general contact channels.
Treatment Credits and Gift Certificates
Prepay for a set amount of treatment credit to use toward eligible services. Treatment Credits are units of eligible care and are recorded for your account at the selected participating clinic. Current packages are 6 credits for $427.50; 12 for $832.50; 24 for $1,575.00; and 48 for $2,970.00. The credits and price accepted at purchase are retained with that transaction.
Treatment Credits are specific to the account holder. Ordinary direct person-to-person transfer of Treatment Credits is not available. To share credits, create a transferable Gift Certificate where offered. Only an authorized clinic or support representative may approve and record a transfer between participating clinics; contact support@innervital.com for assistance.
Treatment Credit purchases and Gift Certificates are non-refundable except where required by applicable law. Treatment Credits and Gift Certificates never expire and have no inactivity reduction. They are not cash, a stored dollar balance, or a guarantee of appointment availability or treatment appropriateness.
In-person Acupuncture/TCM and Massage/Bodywork use 2 Treatment Credits per treatment. Acupuncture/TCM telehealth uses 1 Treatment Credit. Massage/Bodywork is available in person only. Cash payment choices are $160 in person or $80 telehealth at treatment, or $144 in person or $72 telehealth when paying now. Credits cannot be automatically combined with cash to pay for a treatment.
Creating a Gift Certificate immediately removes its credits from the sender’s available balance. A Gift Certificate is a transferable bearer certificate: anyone holding the code may redeem its full unused value once into an authenticated account at the originating clinic, regardless of the intended recipient name. Keep the code private. Lost or stolen codes require support review; replacement is not guaranteed. Delivery failure does not invalidate the certificate.
Credit-paid appointments debit the associated clinic wallet when successfully scheduled. Eligible cancellations restore credits through an additional ledger entry under the existing Clinic cancellation/refund policy; they do not refund the original package purchase. Rescheduling adjustments depend on treatment modality and clinic.
Transfer of remaining unused Treatment Credits to another participating InnerVital clinic may be available upon request. Please contact support@innervital.com. A request is subject to approval; patients cannot move credits between clinics themselves. Approved transfers preserve total credits and are recorded in both clinic statements.
The professional practice shown for the selected clinic provides the clinical care and receives its clinical payment. These policies do not limit rights required by applicable law.
